The sanctuary is set in a cave in the town of Monte Sant'Angelo, on the Gargano peninsula in Apulia. According to tradition, at the end of the 5th century Saint Michael the Archangel appeared here to the bishop of Siponto and himself consecrated the cave as a place of prayer. In the Middle Ages the Lombards supported the sanctuary, and today it is part of a UNESCO World Heritage site.

Pilgrims descend a long stairway to the cave, called the Celestial Basilica, to pray to the Archangel for protection and strength in spiritual struggle. For centuries travellers heading to the ports of Apulia and on to the Holy Land came here, and the sanctuary was an important stage on medieval pilgrim routes.

The main celebrations fall on 8 May, the anniversary of the apparition, and on 29 September, the feast of the Archangels. The nearest larger city is Foggia.
